All forms must be completed and signed by the child’s parent or legal guardian.

If you’re already registered and need to make changes, you do not need to fill out this form again; give us a call, and we can update it for you.
Please note you will NOT receive an email confirmation once you have completed this online registration. Your information will go directly into our database.
We conveniently allow you to sign our required paperwork electronically. Choose the form corresponding to the number of children in your family and sign accordingly.
The state of Florida requires that we have the yellow and blue physical and immunization forms (or exemption) from your pediatrician’s office.
You can also have your pediatrician email them to us at info@thestopandplay.com.
You do NOT have to have this record for your first visit, but it must be submitted to us within 30 days if you’re planning a second visit.

If a different person is picking up from the ones who are listed in your authorized pick up list, you will need to notify us via email or in writing.
The intention of Stop & Play is for children to interact and connect with other children and discover new games, toys, and environments. For this reason we’re screen-free. No iPads or phones allowed.
